I truly wonder how many behavioral health companies are going to be on the Inc. 5000 list next year.
Every year around this time, we look at the business media’s definitive list of America’s fastest-growing private companies. Each year, I see more behavioral health companies posting notable growth numbers. This year is no exception. I count 52 companies that I would classify as dedicated to the industry, a new all-time high. Last year, I counted 45.
The basic investment dynamics of behavioral health have not changed in the last few years. There are critical access challenges to the industry’s services and imbalances between the demand for the service and supply. The potency of this dynamic varies widely based on location. What has changed a lot is the operating conditions. And they keep changing at an accelerated rate.
Growth — heck, even survival! — is far from guaranteed in this incredibly fluid marketplace. A lot of the changes, such as the impact of the One Big Beautiful Bill Act and the overall rationalization of autism therapy spending, are just now being fully realized. The recently released list of fastest-growing companies looks at a three-year revenue period ending in 2025.
Still, there are a lot of changes that may suggest a shakeup in the industry’s fast growers, including notable companies that are not on the list.

Big companies not on the list
There are several titans of the industry or fixtures of the Inc. 5000 list that did appear in 2026.
From the jump, I’m not saying that this is some kind of bad signal for that company. While helpful to outside observers of American industry, the Inc. 5,000 list is, ultimately, a marketing tool for the companies that get involved. Maybe the company just didn’t prioritize the list this year? Still, when considering all things equal, a company’s size and prior history of being on the list raise questions when they are suddenly not on the list. I’ve often heard it said that marketing and ad budgets are often quick to go when cash gets tight.
Further, startup-like growth figures don’t persist over time, even at healthy companies. That is a phenomenon of companies in their early days.
First to note: Boston-based The Stepping Stones Group. The company has made the list every year for the past decade until this year. That is the ending of an era. It previously ranked No. 4,109 with revenue growth of 87%. If a genuine signal (again, it might not be), there would have had to have been a major contraction within The Stepping Stones Group’s business to not make the list: No. 5,000 on the Inc. 5000 — an I.T. company called ANC Group — made the list with 12% growth of the three years ending in 2025.
To qualify, a company must be an independent, privately held, U.S.-based, for-profit business that earned initial revenue by March 31, 2022, totaling at least $100,000 in 2022 and reaching $2 million in 2025.
A once-category leader is also not on the list this year: Fort Lauderdale, Florida-based ABA Centers of America topped the list of behavioral health companies last year, racking up a 7,755% three-year revenue growth number, a rank of No. 25. The company is working through a realignment and shedding employees after losing access to reimbursement from Optum.
Transformations Care Network is absent from the list. The outpatient mental health provider organization is among the largest in the nation — especially among providers that treat patients with Medicaid.
Ellie Mental Health, compleatKiDZ, Behavioral Innovations and Stepping Stones Behavioral Solutions are also not on this year’s list. Ellie Mental Health is a large mental health clinic franchisor that ran headlong into several lawsuits with franchisees and a cash flow problem in 2025. The latter three in that list are ABA-focused autism therapy providers. Stepping Stones Behavioral Solutions shut down following the ABA reimbursement rationalizations in Indiana. CompleatKiDZ has been scrutinized for its massive growth in Medicaid revenue, especially in North Carolina.
Trends and composition of the list
On an absolute basis, there are now seven more behavioral health companies on the Inc. 5000. But there were 31 companies that were on this list this year but not on the previous list. Said another way, there are 25 previous listees that did not reappear in 2026.
Most of that turnover was among the smaller organizations: 13 of the companies that dropped off the list had $10 million or less in revenue. By my count, 17 of the newcomers fit the same revenue bracket.
Like previous years, mental health companies that operate in-person and virtual services made up the bulk of the list. Twenty-two, or 42%, of the 2026 Inc. 5000 list are mental health companies almost totally focused on outpatient services, especially psychotherapy and medication management providers.
We did not see a behavioral health company crack the top 10. The highest ranking among the cohort was a digital mental health provider for seniors called Total Life. The Jupiter, Florida-based provider ranked No. 41 with 5,989% revenue growth.
Somewhat in line with expectations, many of these fast-growing companies are on the younger side. Newer companies that are in their infancy and grow quickly have impressive growth percentages because their previous year’s number — the denominator — are similarly undeveloped.
